I don't know about timing or how your truck runs under load, but the egr and air idle control valve could be the problem. Have you looked them over?

Makes sense. That's my problem also. I'll get the AC Delco version. I bought a bad egr valve from auto zone last year, so I only go to AC Delco for parts that matter now.

I have the same body style. What advice would you give for a lift kit? I might go 6", I don't mind keeping my gas mileage and only going 2". What brands do you recommend? What would you do differently?

That is exactly what the problem was. I replaced both with Carquest parts and the problem is fixed!
